What is Medical Home Team?
Medical Home Team is a model that we have been using to provide primary care to Medicare patients in their own homes for the last three years. We have two dedicated Nurse Practitioners who make monthly scheduled visits to patients' homes.
What should I expect?
Home visits are like an office visit, but the provider comes to you. A provider will come to your home once a month for an office visit. Medicare covers home visits like they do office visits, which means there is no extra cost for joining this program.
Program Goals
Provide continuity of care, especially when patients see other providers outside of our office (i.e., Urologists, Cardiologists, Neurologists, etc.)
Important Information
Medical Home Team visits are not home health visits. Patients can be seen by home health and our Medical Home Team.
How do I sign Up?
It is simple to qualify for our Medical Home Team. All you have to do is call (405) 806-2200 ext.241 and ask to speak with Stephanie about scheduling a Medical Home Team visit.

Environmental allergy skin testing.
Treatment options include allergy injections and sublingual immunotherapy.

We offer convenient and cost-effective self-pay office visits and lab testing. Please contact our office to confirm if we accept your health insurance.
Rapid Covid test cash price is $75
We understand that life is unpredictable and emergencies may arise that require you to miss your appointment. However, when you don't call to cancel your appointment, you may be preventing another patient from receiving much needed treatment. Likewise, if another patient fails to cancel their appointment, we may be unable to accommodate you due to a full schedule. If an appointment is not canceled in advance, you will be responsible for a $20 fee that is not covered by insurance.
